Would you like to be part of Adult Social Care, offering support and guidance to residents of Doncaster and their families in the North of the city. We strive to maintain a person centred approach that enables individuals to identify their own strengths, what is important to them, what support they may require and how to maintain their independence wherever possible, while working closely with both internal and external partners.

A fantastic opportunity has arisen to join the Adults Wellbeing and Culture Directorate in the North Team as a Community Care Officer. Undertaking initial conversations and assessments for people and carers who need support, promoting independence and reducing risks for adults living in Doncaster in line with the Care Act (2014). Applying person centred, strengths based approaches alongside other professionals to identify what support is needed whilst respecting individual values, culture and diversity. Your role will also include safeguarding vulnerable adults when the need arises, working closely with others for support.

Essential skills and knowledge in recording and report writing, strength based approach to assessment with an aim of positive outcomes, awareness and experience of risk assessment are key to the role. You will also work with carers to support, signpost and assess their needs in accordance with the Care Act 2014. Knowledge of current Adult Social Care Policy and legislation is key.

As a Community Care Officer you will also support and assist in the development and implementation of the locality based community led support model using creative ways to support service users and carers.
