Job Description
**Overview:**
We are looking for a committed and compassionate Care Manager to join our team. In this role, you will oversee and coordinate the care of individuals within a home care environment. The ideal candidate will possess excellent communication skills and a proven ability to develop and implement effective care plans.

**Responsibilities:**
– Develop and implement personalized care plans for each individual
– Coordinate with healthcare professionals, family members, and caregivers to ensure quality care
– Monitor and evaluate the effectiveness of care plans regularly
– Provide emotional support and guidance to individuals and their families
– Maintain accurate records of all care provided
– Utilize IT systems for documentation and communication purposes
– Drive to different locations as needed
**Qualifications:**
– Experience in home care or a care home setting is preferred
– Required Health and Social Care – Level 5 and above.
– Proficient in English to effectively communicate with individuals and their families
– Strong interpersonal skills to build relationships with clients, families, and healthcare providers
– Ability to create detailed care plans tailored to individual needs
– Valid driver’s license for transportation needs
