The person will have experience in the care sector as well as knowledge of scheduling home carers to ensure all clients receive a good quality visit.

Responsibilities include;

Ensuring that Regulatory Requirements are met.

Implementing a high quality service through effective matching of service user needs with suitably qualified and experienced care workers. Liaising with the Registered Manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion.

Ensure working patterns are efficient and planned logically.

Prioritise tasks to meet unexpected emergencies and changes in care packages.

Maintain clear and accurate written records and follow statutory reporting procedures.

Communicate clearly and build positive working relationships with clients, their families and other social care professionals as appropriate.

Receive and handle complaints about the service from service users or others.

Excellent communication skills and the ability to lead a team of home carers.

Role Overview: We are currently seeking a skilled and compassionate Care Coordinator. In this role, you will be instrumental in managing a team of support workers and healthcare assistants, ensuring the highest standards of care for clients. This would be a great progressive route for a future RM.

Key Responsibilities:

  • Line manage a small team of support workers and healthcare assistants, providing necessary support during their employment.
  • Directly manage the care of allocated clients, emphasising positive outcomes and active participation.
  • Oversee client visits, conduct office work and consultations, and perform service reviews and quality assurance visits.
  • Undertake supervisions with support workers and healthcare assistants, including mentoring checks.
  • Ensure adequate staff cover for timely care and support delivery.
  • Monitor carer allocation, maximizing efficiency through call monitoring systems.
  • Collaborate with the office team to maintain accurate client records.
  • Resolve conflicts arising from clients, staff, and stakeholders in a prompt and effective manner.
  • Learn to operate rostering software efficiently.
  • Handle short-term roster changes and plan regular visit schedules for new clients.
  • Utilize your experience in a similar role with qualifications in Health and Social Care.
  • Think on your feet, juggle priorities, and demonstrate flexibility with excellent telephone manners and computer skills.
  • Possess the right to work in the UK, a full driving license, and full access to a car. Being on the DBS update service is an added advantage.
  • CQC Knowledge also an advantage

Job Description

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly/children and families and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. Care coordinators review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

Care coordinators could potentially provide time, capacity and expertise to support people in preparing for or following-up clinical conversations they have with primary care professionals to enable them to be actively involved in managing their care and supported to make choices that are right for them. Their aim is to help people improve their quality of life. The successful candidate will be based in a local cluster of General Practices as part of the Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people.

They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support. This role is intended to become an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN. There may be a need to work remotely depending on the requirements of the role.
