Purpose of Role
We are looking for someone who can lead by example and provide high-quality person-centred care and support in a supported living setting for individuals with complex care needs, including autism and behaviours of concern. You will lead and manage a set cluster of Moorview Care’s supported living services, ensuring the team delivers safe, compliant and effective care and support.
Eligibility Requirements

- Level 3 qualification in H&SC or above, Level 5 desirable.

- Enhanced Disclosure from the Disclosure and Barring Service.
- Desirable – Full driver’s licence with no more than 6 points and Class 1 business insurance (expenses paid for insurance where necessary).
Day to Day Duties & Activities
- Facilitate Co production with our clients including the participation of the development of Moorview Care.
- Drive best practice in Services. Implement best practice initiatives.
- Maintain relationships with key external professionals and families.
- Attends and actively participates in regional forums representing Moorview Care.
- Support with the opening of new service and increased commissioned hours.
- Contribute to growth plans, highlighting Service strengths and weaknesses that may impact growth.
- Review daily living support needs for clients and attend client reviews.
- Responsible for coordinating all clients have a HAP with regular reviews and health checks as required.
- Responsible for coordinating clients’ Communications Passport who require one.
- Responsible for communicating client goals, then checking progress.
- Review support plans, contributing as appropriate, to ensure they are fit for purpose and support the needs of the client.
- Responsible for Support Plan sign off.
- Coordinate and attend client reviews.
- Responsible for ensuring the appropriate preparations have been done in readiness for any client review.
- Support the creation and implementation of transitions.
- Participate in assessments, attend discharge meetings and CPA/MDT reviews.
- Responsible for the creation of the PBS plan, ensuring they are fit for purpose and support the needs of the client.
- Review of PBS reports and completion of Behaviour Incident Reports.
- Undertake team debriefs in line with risk.
- Approve monthly finance audits and set actions required.
- Complete quarterly audits on client finances and expenditure.
- Approve and sign off client Risk Assessments.
- Write and review Risk Assessments for high risk scenarios and situations.
- Complete quality assurance checks of accurate Service record keeping in line with QAF.
- Deliver a service in line with the Moorview Care Standard, our code of conduct and overall quality and performance expectations.
- Undertake health & safety checks, reporting any concerns and repairs where required
- Contribute to RIDDOR reports.
- Undertake reporting required by the Landlord.
- Ensure team compliance and due diligence to all policies and procedures.
- Contribute to the Single Assessment Framework Report.
- Follow safeguarding procedures and action safeguarding concerns.
- Attend Safeguarding planning meetings and client case meetings as needed.
- Undertake meds competencies, error investigations and audits for medication records.
- Attend client medication reviews where required.
- Monitor and report any building repairs, maintenance or concerns.
- Undertake driver checks to ensure the team are eligible to drive the company vehicles.
- Report data protection and confidentiality breaches in line with procedure.
- Implement actions and procedures where necessary.
- Undertake complaints investigations in line with procedure.
- Implement any outcomes from complaints or concerns.
- Actively participate, create an agenda, and take notes for team meetings, chair where appropriate.
- Attend Operational Management Meetings.
- Responsible for constructing a suitable rota.
- Review commissioned hours, review and complete the rota, authorising changes.
- Monitor progress and ensure completion of induction of new starters and the Job Induction Programme documentation.
- Write and deliver probation reviews as required.
- Monitor and update the People Compliance Tracker.
- Conduct supervisions, ad-hoc documented discussions, team debriefs, annual performance & development reviews (APDR’s), personal development plans (PDP’s), Return to Work Interviews (RTWI) and any other 1-2-1 discussions with the team where required.
- Be part of a regional on-call rotation team.
- Responsible for the management of annual leave.
- Responsible for recruitment; chair interviews, and monitor resourcing for Services
- Responsible for ensuring team members are suitably trained and training is booked and recorded in a timely manner.
- Monitor the Training Matrix to ensure team training compliance.
- Contribute to the Learning Needs Analysis documentation.
- Monitor resourcing hours for payroll purposes, highlighting any discrepancies as required.
- Complete checks on billable and nonbillable hours.
- Ensure the rota system accurately reflects billable hours.
- Monitor and actively promote efficiencies, demonstrate financial awareness and be accountable for Service spend.
- Complete a monthly petty cash audit.
- Be able to drop in to perform all duties of all members within the team.
- Have an awareness of and act in accordance of CQC requirements.
- Support with CQC Inspections.
- Contribute to the PIR as required.
- Contribute to project proposals for annual business planning.
- Contribute as a collaborator to the delivery of projects associated with the delivery of our Strategic Priorities and Objectives.
