Job Title: Access Wellbeing Co-ordinator
Hours: 37.5 hours a week
Responsible to: Senior Wellbeing Co-ordinator
Contract: Permanent
Salary: £25147 – 27596
Location: Must be within easy distance of Shaftesbury or North Dorset Area; Remote Working
Dorset Mind are working with Partners to deliver wellbeing hubs, across Dorset. This is part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs will support people to take the steps that matter to them, by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities. Citizenship in this context means having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.
Position
At our hub, you’ll create a welcoming environment where people feel safe and valued. You’ll build rapport and use a strengths-based approach, recognizing individuals as experts on themselves. This role involves enabling choice, advocating for individuals in multi-disciplinary meetings, and knowing when to escalate concerns.
You’ll promote local resources and well-being opportunities, helping people advocate for themselves. You’ll develop personalized learning and well-being plans, focusing on safety and utilizing community resources. Support will be provided through various communication methods, ensuring confidentiality and effective care planning.
By addressing health inequalities and engaging communities, you’ll build essential partnerships with local professionals. Accurate record-keeping and effective caseload management are key. You’ll encourage active citizenship, engage with communities, and commit to ongoing learning and reflective practice. Join us to make a real difference in people’s lives!
