Job Description
Purpose of the role:
To advocate for patients to make sure they get high quality care, perform a range of administrative tasks, such as entering service user data, answering phone calls and scheduling service user visits. The role involves a significant amount of communication with service users, their family members, and various Healthcare Professionals. The care coordinator synthesises information form these people, such as service users needs and healthcare goals, in a way that’s useful and helpful to the service users and healthcare professionals.
Main Responsibilities, Task and Duties:
- Accepting care packages and assigning care workers to service users in an appropriate manner.
- Ensuring that service user’s files are complete.
- Arranging care reviews and risk assessments for service users.
- Creating weekly audits and writing weekly reviews of service users and the service.
- Conducting telephone monitoring and service user visits and ensuring service quality, liaising with clients, local authorities, and other stakeholder.
- Ensuring care assessments, care and risk planning meets best practice and service user aspirations.
- Creating, managing, and monitoring customer care plans, ensuring staff rotas meet residents’ needs and aspirations.
- Managing care workers and conducting care worker supervisions.
- Representing the company during meetings with officials and service commissioners.
- Attending training events and ensuring that skills and knowledge of industry best practice is kept up to date.
- Completing medication audits.
- Monitor, supervise and mentor all Care Workers in the field;
- Ensuring that the service complies with CQC requirements and ensuring the highest standards of care are maintained at all times.
- Work within company Supported Living policies and procedures
- Work in a way that meets the statutory requirements of employees under health and safety at work.
- At all times work within current legislation
- Responsible for employee supervision, probation and appraisal.
- Attending training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.
- To work closely with other members of the team for the ultimate benefit of the clients.
- To ensure that all the receipts and any accompanying paper works are always done and kept up to date.
- To ensure there arises no financial discrepancies.
- Ensure that all written reports and administration is up to date;
- To Keep updated with all financial policy and procedures.
- To work flexibly on a rota basis to meet the needs of the service, this can include weekends.
- Ensure work is recorded accurately and appropriately in compliance with the company requirements and that records made, and personal information used are in compliance with the Data Protection Act and the standards of information governance.
Personal Specification:
- Minimum of 2 years experience required
- Be proactive, innovative, open, and honest as well as trustworthy.
- Be willing to attend any training required to be competent to fulfil your job role.
- An excellent communicator with strong problem-solving skills and ability to think outside the box.

Job Type: Full-time
Pay: £25,000.00-£28,000.00 per year
