Job Description

Purpose of the role:

To advocate for patients to make sure they get high quality care, perform a range of administrative tasks, such as entering service user data, answering phone calls and scheduling service user visits. The role involves a significant amount of communication with service users, their family members, and various Healthcare Professionals. The care coordinator synthesises information form these people, such as service users needs and healthcare goals, in a way that’s useful and helpful to the service users and healthcare professionals.

Main Responsibilities, Task and Duties:

  • Accepting care packages and assigning care workers to service users in an appropriate manner.
  • Ensuring that service user’s files are complete.
  • Arranging care reviews and risk assessments for service users.
  • Creating weekly audits and writing weekly reviews of service users and the service.
  • Conducting telephone monitoring and service user visits and ensuring service quality, liaising with clients, local authorities, and other stakeholder.
  • Ensuring care assessments, care and risk planning meets best practice and service user aspirations.
  • Creating, managing, and monitoring customer care plans, ensuring staff rotas meet residents’ needs and aspirations.
  • Managing care workers and conducting care worker supervisions.
  • Representing the company during meetings with officials and service commissioners.
  • Attending training events and ensuring that skills and knowledge of industry best practice is kept up to date.
  • Completing medication audits.
  • Monitor, supervise and mentor all Care Workers in the field;
  • Ensuring that the service complies with CQC requirements and ensuring the highest standards of care are maintained at all times.
  • Work within company Supported Living policies and procedures
  • Work in a way that meets the statutory requirements of employees under health and safety at work.
  • At all times work within current legislation
  • Responsible for employee supervision, probation and appraisal.
  • Attending training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.
  • To work closely with other members of the team for the ultimate benefit of the clients.
  • To ensure that all the receipts and any accompanying paper works are always done and kept up to date.
  • To ensure there arises no financial discrepancies.
  • Ensure that all written reports and administration is up to date;
  • To Keep updated with all financial policy and procedures.
  • To work flexibly on a rota basis to meet the needs of the service, this can include weekends.
  • Ensure work is recorded accurately and appropriately in compliance with the company requirements and that records made, and personal information used are in compliance with the Data Protection Act and the standards of information governance.

Personal Specification:

  • Minimum of 2 years experience required

You will be working alongside other Care coordinators and supervisors reporting to the Registered Manager and Operations Manager. As a Care Coordinator, you will use your excellent communication and leadership skills to build relationships with our carers, clients, and their families. You will be responsible for ensuring that high standards are maintained throughout the company and that clients receive great quality care. The successful candidate must be willing to carry out hands on care to clients in an emergency.

Person Specification

A self-motivated and enthusiastic individual, with a positive attitude and pro-active approach.

This person must be extremely reliable and be committed to this role of high responsibility, flexibility is essential to meet the demand of the service which can change at short notice.

They must have a caring nature and the ability to understand the changing needs of the service users and the dynamics of thecare industry as a whole, including staffing, environment, target achieving, planning and the legal aspects of the industry.

The individual will be competent at dealing with people on all levels and have the ability to think quickly and act accordingly. This person will be adaptable and take a non-judgmental approach to encompass a thorough understanding of the life changing effects of care and be able to deal with both positive and negative feedback and outcomes.

Applicants must have a full driving licence.

The successful candidate will be responsible for rota management and shift cover, care planning, risk assessments, staff management and comply with all of Eildon’s policies and have a working knowledge of the care inspectorate, the care standards and the SSSC codes of practice.

Job Description

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. Care coordinators review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

Care coordinators could potentially provide time, capacity and expertise to support people in preparing for or following-up clinical conversations they have with primary care professionals to enable them to be actively involved in managing their care and supported to make choices that are right for them. Their aim is to help people improve their quality of life. The successful candidate will be based in a local cluster of General Practices as part of the Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people.

They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support. This role is intended to become an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN. There may be a need to work remotely depending on the requirements of the role.
