Job Description

Overall Purpose of Role: To create and support a homely and welcoming environment in which families feel safe nurtured and rested. Provide care (physical, emotional, spiritual and social) to the affected child and their family. To work flexibly both within Martin House and the community providing Hospice @ Home. Tasks and duties: 1.

To contribute positively to the creation and help sustain the welcoming community environment, and to establish good communication. 2. To provide total holistic care to the referred child, including activities such as play and recreation as well as attending to the childs physical and emotional needs. 3.

To hold each member of the team in positive regard and to foster a culture of respect and consideration between all staff. 4. To work flexibly alongside families respecting the childs routine of care. 5.

To ensure that care routines and practice are documented accurately and are clearly communicated. 6. To participate equally with other care team members in general household activities to facilitate the smooth running and family feel of Martin House. 7.

To ensure accurate and contemporaneous record adhering to standards of confidentiality at all times. 8. To work closely with the Care Team Leader on each shift and inform them of any issues that need resolving. 9.

Non-medical prescribers only: To undertake prescribing for children and young people with a range of palliative care needs, within your scope of competence, if authorised to do so. Health & safety: 10. To be aware of the safety needs of the children, siblings, parents and colleagues and adopt a preventative safety approach to all times. 11.

To report all accidents incidents and untoward incidents to a Deputy Director of Clinical Services. 12. To be aware of your health and safety responsibilities as an employee and adhere to these. Personal development: 13.

To adopt a positive and reflective approach to personal and professional development. 14. To participate constructively in a yearly annual appraisal. 15.

To undertake core competencies for clinical skills and specific training and to remain updated. 16. To work in conjunction with the education co-ordinators to develop your practice and care for the children. 17.

To work contracted hours including unsocial hours as required, weekends. evenings and nights. Nursing standards Nurses will be expected to abide by the following standards 18. To adhere to the law and adhere to practice in accordance with the NMC.

19. To accept responsibility for ensuring the safe and correct administration of medicines. 20. To ensure they are or commit to becoming competent in the identified clinical skills and to take responsibility for maintaining the skills.

This job description reflects the present requirements of the post and will form the basis of performance appraisal. As duties and responsibilities change and develop the job description will be reviewed and necessary changes made with consultation. Care team members will be required work in the community and therefore will need drive a Martin House pool car (or have access to their own vehicle for business purposes). Job Description

Martin House has been caring for babies, children and young people with life-limiting conditions from West, North and East Yorkshire for more than 30 years. Every year we care for more than 420 families at our hospice in Boston Spa, in hospitals and in their own homes. We also support more than 150 bereaved families each year.

We are seeking Care Team Nurses and as valued members of the clinical care team, you will join a unique, award winning and vibrant team providing symptom management, palliative and end-of-life care, as well as planned and emergency short breaks both at the hospice and in the community.

We offer 1:1 nursing and the opportunity to working as part of the MDT team made up of a variety of skilled members including art and music therapists and a 24 hour medical team. In return, we will support your wellbeing and offer you a learning environment to develop your skills and competencies in paediatric palliative care, supported by our Nurse Practice Educators.

We are looking for compassionate and friendly candidates with a children’s nursing or learning disabilities qualification who have a significant level of hands-on post qualification experience.

  • Will need to have knowledge of current legislation relating to children and young people including best practice in safeguarding children.
  • Will support the wider team to ensure high standards of holistic care are delivered.
  • Be able to assess, plan, implement, evaluate, care and end of life care plans to meet the diverse and individual needs of children, young people and families.
  • Has a genuine interest in making a difference, in palliative care with children and young adults.

  • Be willing to take part in flexible shift work to include early mornings, evenings and weekends.
