Job Description

Macklin Care Homes are currently recruiting Senior Care Assistants to join their friendly, caring, and progressive Ratheane Care Home, Coleraine working 22hrs per week. This is a vital role within the Home, working as a team to lead the delivery of excellent care to much loved residents.


Responsibilities
:
Lead and work collaboratively with the care team to deliver excellent quality, person-centred care to all residents
Enrich the lives of residents through the organisation and delivery of activities
Proactively encourage and develop team members
Ensure all documentation is completed in line with Home procedures
Assist inducting new team members into their role and the Home
Advocate and promote the company mission, vision and values
Supporting the nursing team to ensure compliance with policies and procedures


Ideal candidate:

Minimum one-year caring experience in a Care Home environment
Passed probation period within current role in Macklin Care Homes
Excellent leadership skills, with the ability to lead and develop a team
Exceptional communicator at all levels

Principle Tasks:

1.1 People Planner and Access Care Planning

You will conduct and devise person centered care plans on access care planning within the contractual guideline. You will contact customers and/or representative and book assessment visit within (1hr) and conduct home/hospital visit within (24hrs) to carry out full assessment and ensure a schedule of reviews are undertaken at six- or twelve-monthly intervals.

Person Centered Care Plans should include as a minimum a person’s strengths & assets, how they will regain/increase/maintain confidence in their independence; personal details, key contacts, friends & family, inter-agencies, health history, religion, sexual orientation, hobbies & activities, medication, mental capacity, visit schedule & care tasks, risk assessments.

1.2 Mentoring

To lead by example and promote Diamond Home Care’s values and responsibilities to care workers by organising, supporting and maintaining the Shadow Diary & Integration of new Community Staff by providing detailed customer care plans and appropriate on-the-job support to ensure that they can implement the 15 Units of the Care Certificate Induction Program into their delivery of tasks within the packages of care.

1.3 Business Growth

Assist in business growth and development of services by completing assessments and visits of new packages of care until such time that they are fully assessed and handed over to a structured round and templated to a Community Care Worker.

1.4 Key Worker Support

To support the care co-ordinator to identify and facilitate any carer key workers. The support key workers to report changes of circumstances of customers needs transparently.

To cover scheduled holiday requests and absences cover, and to prevent any possible missed calls within your service area, by covering calls at very short notice to minimise complaints and/or concerns.

1.5 Out of Hours

To participation in the Rota of the Out of Hours Service, between all the Seniors within your area of responsibility. Ensuring that all records are maintained and completed contemporaneously, (People Planner – Alerts & Tasks and OOH Daily Logs) ensuring the you bring to the attention of your Line Manager any Medication Errors, Incidents or Accidents.

1.6 Professionalism

To promote a professional image and cultivate an open, transparent, and supportive working environment. Support the building of relationships on behalf of Diamond Home Care with local authority brokerage and other relevant teams, by providing statistical data pertaining to employee availability and surplus service capacity. Also ensuring that any temporary or short-term packages are extended to full- time provision.

1.7 Rostering

To support the Care Co-Ordinator to create and maintain staff schedules using People Planner, by providing up-to-date and concise information pertaining to staff absences and management of staff holiday requests.

1.8 Quality Assurance

– To conduct quality assurance visits with customers via home visits to complete the Service Users Reviews. Action all feedback in a swift, effective, and in a professional manner, taking guidance from your Care Co-Ordinator.

– To complete Meeting Record(s) with Community Care Staff, Inter-Agencies, Family & Friends and/or Contracting organisations regarding any queries or incidents that are not outlined within the People Planner Rostering Management Software.

– To participate and co-ordinate Quality Assurance Audits (Staff Spot Checks & Medication Audits) within your areas of responsibility on the Community Care Staff in-line with the Care Quality Commission Regulatory requirements and internal Quality Assurance Auditing schedules.

1.9 Complaints

To support your Line Manager with the Complaints, Concerns, and Compliments procedures by participating in and/or carrying out investigations in the community.

About the Role :

As the Senior Care Assistant, you will be tasked with managing the care team within the Home and ensuring that care staff are adhering to CQC guidelines whilst providing exceptional levels of care to residents. Job Description

This role consists of a combination of office and community-based tasks, which will be dictated by the day to day and strategic needs of the service and allocated by the management team as required.

  • To provide front line, Community based personal and practical support visits to Customers as required by the needs of the service on a day to day basis to ensure quality and continuity within services. Customer calls in the community will be scheduled as required and will take precedent over other tasks. This could be a combination of morning, lunch, tea and PM calls.
  • To conduct spot checks and observations on Community Based staff as required by the Operations Team and HR Team and in line with agreed KPI’s.
  • As part of a team, to undertake new and existing Customer Risk and Needs Assessments and support the implementation of new support packages (including direct care delivery) until incorporation into existing staff rotas.

  • To provide expert guidance in relation to resolving specific difficulties that staff may experience in the community. This may involve visiting staff in the community to provide Customer specific, or environment specific guidance, or demonstrations in the safe use of specific equipment.
  • Under the guidance of the management team, to gather information as required in relation to Customer complaints, issues or staff investigations.
  • To deliver the new and existing staff training and mentoring programme. This will include ‘classroom’ based training according to training sessions plans.
  • To provide crucial on-going and daily support to the Care Management team to ensure that our Customers receive a quality and consistent service.
  • To review and undertake appropriate action in relation to the daily, area CM reports and communicate relevant information to the area Operations Officer.

  • To maintain awareness of issues and difficulties relating to Customers support and report any on-going issues/concerns to the management team, as appropriate.

  • To be responsive to the needs of our community-based staff team, providing critical support and guidance on issues as they arise, either remotely or through attending visits in the Community.
