Job Description

Purpose of Role

As a Senior Practitioner, you will be entrusted and required to provide a high standard of care to all Clients, by respecting and upholding their rights, choice, privacy and dignity. As well as providing first-class care, you will manage a team of Community Care Practitioners, you will motivate and support CCP through their induction, and supervision sessions, and monitor and mentor their day to day performance. It is your duty to ensure our ‘Divine’ Clients are empowered to be as independent as possible, carrying out your responsibilities and duties effectively. Senior Practitioners are required to comply with Clients’ Personal Care Plans which detail and describe the care to be provided.

Key Responsibilities:

  • To provide the highest quality care to Clients in their own homes in accordance with their care plan, in a person-centred approach.
  • Work alongside management and participate in the management of the service and the supervision of other staff.
  • Manage, motivate and support Care Assistants through company induction, shadow shifts, training and guidance as and when required with day to day tasks.
  • Maintain close contact with Management with regards to the level of service provided to Clients and changes in their circumstances.
  • Ensure risk assessments and care plans are completed and updated in line with the company procedures.
  • Report any concerns regarding the well-being of Clients or the level of service being offered to allow Management to re-assess their situation.
  • Undertake and assist with personal care including; washing, dressing, bathing and toileting.
  • Assist with grooming such as brushing hair, shaving and dental hygiene etc.
  • Administer, prompt and assist with medication you have to ensure medication is taken correctly and the MAR sheet is completed correctly in conjunction with Divine’s procedures.
  • Assist Clients with preparing and cooking meals, essential cleaning and housekeeping.
  • Undertake any necessary training provided by Divine.
  • Carry out supervisions and appraisals in a timely manner.
  • Actively participate in regular supervisions and appraisals.
  • Ensure that the hours and times of visits to Clients are kept to unless authorised by Management.
  • To immediately notify Management of anything, which prevents you from carrying out your work, keep in regular contact and confirm when you are ready to resume work, as cover will have been arranged in your absence.
  • Work together and cooperate with colleagues and other agencies involved in the assessment of Clients’ changing needs.
  • Encourage Clients to maintain contact with their family and friends to encourage their involvement and be sensitive to their limitations.
  • Attend relevant meetings and assist in the making of recommendations about the abilities of Clients and their future care plans.
  • Support Management in specific areas, such as medication, assessments, care plans and care reviews.
  • Ensure Clients’ notes of service are updated each visit reporting any problems to Management if required
  • Cover On Call on a rotational basis.
